NEW PATIENT WELCOME PACKET
To assist in making your initial visit about you and not paperwork, we have made available online this welcome packet for you to complete prior to your new patient appointment. If you have any questions regarding any of the forms, please ask the front office receptionist to assist you when you arrive for your appointment. We also ask that you bring any previous scans or medical files that may assist in a full diagnosis of your scan.
